[摘要]目的:探讨两种材料的全冠修复体对低牙合龈距年轻恒磨牙牙周组织的影响。方法:选择24例低牙合龈距年轻恒磨牙分别采用钴铬合金全冠、金铂合金全冠进行修复,全冠戴入4周后复诊,分别检测修复后4、12周各项牙周指标及患牙的龈沟液量(GCF)。结果:修复前和修复后4周时,两组间各检测指标无明显差异,修复后12周时钴铬合金全冠组的GCF量、菌斑指数、龈沟出血指数均高于修复前、修复后4周和金铂合金全冠组。结论:临床合格的钴铬合金全冠对低牙合龈距年轻恒磨牙牙周组织在短时期内有一定不利影响,金铂合金全冠则影响相对较小。

Abstract:ObjectiveTo evaluate the influences of two crowns materials on periodontal tissue for the frist permanent molar with lower occlusogingival distance.MethodsTwenty-four frist permanent molar with lower occlusogingival distance were divided into two groups randomly for Co-Cr alloy crown and Au-Pt alloy crown.After crowns placement for 4 weeks,the gingival crevilar fluid(GCF)sample were collected and weighted,then the clinnical parameters such as sulcus bleeding index(SBI),plaque index(PLI)were recorded.ResultsBefore tooth preparation and 4 weeks after cementation,all indexes showed no difference among two groups.The level of GCF,SBI,PLI of Co-Cr alloy crown showed conspicuous difference with Au-Pt alloy crown after 12 weeks. ConclusionCo-Cr alloy crowns being clinically accepted have negative influences on gingival health of the restored teeth. Au-Pt alloy crowns may affecct periodontium less than the Co-Cr alloy crowns in observation of this study.

牙合龈距过低年轻恒磨牙的修复一直是临床难题,我们运用纤维核+全冠修复结合Dahlappliance(简称Dahl) 氏方法进行治疗,取得较好疗效,但不同修复材料对患牙牙周组织有何影响,在以往的研究中很少提及,本研究通过对修复前后患牙的牙周指标和龈沟液量进行检测,了解不同修复材料对低牙合龈距年轻恒磨牙牙周组织的影响,以期更好地指导临床修复选择和制作。
1资料和方法
1.1临床资料:选择2006年3月～2008年3月到我院口腔科就诊的牙合龈距过低牙体严重缺损的年轻第一恒磨牙患儿24例,共24颗患牙,患儿年龄10～14岁,其中男性12例,女性12例;上颌牙9颗,下颌牙15颗,均为单侧龋坏。病例纳入条件:①临床牙冠低于3mm、牙体缺损达50%～75%,不能常规修复治疗;②根尖发育完全且经过完善根管治疗、牙根长度≥7mm、牙龈上有≥2mm的牙本质肩领、剩余牙体厚度大于1.5mm、无明显根尖周病变;③牙周检查:口腔卫生状况好,无明显牙周疾患;④患儿全身健康情况良好,能配合治疗,有一定的经济承受能力,家长知情同意。根据冠修复材料不同分为两组:钴铬合金组和金铂合金组,每组各12颗 ......
