Meek植皮术修复患儿大面积烧伤创面的效果研究(1)

     [摘要]目的:探究患儿大面积烧伤创面经Meek植皮术修复后的临床效果。方法:随机选取2016年4月-2017年4月笔者医院收治的110例大面积烧伤患儿,按照数字表法随机分为2组,每组55例患儿。对照组给予自体微粒皮移植术,观察组采用Meek植皮术修复治疗,观察并比较两组患儿的植皮修复效果。结果:观察组患儿手术时间、皮片融合时间均明显低于对照组,且观察组患儿皮片存活率明显高于对照组,差异均有统计学意义(P<0.05)。观察组患儿总有效率为98.18%明显高于对照组的78.18%,差异有统计学意义(P<0.05)。结论:Meek植皮修复术治疗患儿大面积烧伤创面临床效果显著,能够缩短手术时间、皮片融合时间,提高皮片成活率,提高了患儿的生存质量。

    Effect of Meek Skin Grafting on Children with Large Area Burn Wounds

    Abstract: Objective To explore the clinical efficacy of Meek skin grafting for the treatment of children with large are burns wounds. Methods 110 children with large area burn wounds who admitted in our hospital from April 2016 to April 2017 were selected and randomly divided into the control group and the observation group, 55 cases in each group. The control group received particle skin transplantation, and the observation group received Meek skin grafting. The skin grafting repair effect of two groups was observed and compared. Results The time of operation and the time of skin graft fusion in the observation group were significantly lower than that of the control group, and the survival rate of the skin slices of the children in the observation group was significantly higher than that of the control group, the differences were statistically significant(P<0.05). The total effective rate of the observation group was 98.18%, which was significantly higher than of the control group(80.00%), the difference was statistically significant(P<0.05). Conclusion Meek skin grafting repair has a significant clinical effect in the treatment of large area burn wounds in children. It can shorten the time of operation, the time of skin graft fusion, improve the survival rate of skin slices, and improve the quality of life of the children.

    小兒烧伤是临床上常见的皮肤损伤性疾病,由于儿童的安全防范意识薄弱,常因意外损伤、与高温物体接触等因素出现烧伤情况[1]。大面积烧伤是指烧伤总面积在31%~79%或Ⅲ度烧伤面积在21%~49%,当大面积烧伤后,皮肤长期暴露在空气中,易出现交叉感染及体内环境紊乱的情况,严重患儿甚至出现脓毒症,诱发多器官功能衰竭,对患儿身体健康造成一定的伤害,甚至出现系列心理及精神问题[2-3]。常规修复大面积烧伤患儿的治疗方式,多采用微粒皮移植修复术及邮票植皮,创面愈合瘢痕常呈条索状,影响美观[4]。Meek是临床上一项新型机械化微型皮片移植技术,在修复大面积烧伤创面中应用较好,临床效果显著。本研究随机选取笔者医院收治的110例大面积烧伤患儿,按照数字表法随机分组,对比Meek植皮术及自体微粒皮移植术两种修复方式的效果,为临床提供可靠理论依据,以促进患儿康复,减轻痛苦,现报道如下。

    1 资料和方法

    1.1 临床资料:选取2016年4月-2017年4月笔者医院收治的110例大面积烧伤患儿。纳入标准:患儿烧伤面积为31%~79%或Ⅲ度烧伤面积在21%~49%。排除标准:①患有心、肺、肝、肾功能不全者;②具有严重基础疾病患儿。按照数字表法随机分为对照组和观察组,每组55例。对照组:男30例,女25例;年龄1~13岁,平均年龄(5.42±4.53)岁;烧伤面积(65.89±6.33)%;烧伤后入院时间(2.42±1.22)d。观察组:男28例,女27例;年龄1~14岁,平均年龄(5.76±4.39)岁;烧伤面积(67.49±5.78)%;烧伤后入院时间(2.78±1.18)d。两组患儿在年龄、性别、烧伤面积及烧伤后入院时间等一般临床资料方面无明显差异(P>0.05),具有可比性。此次研究经本院医学伦理委员会研究后一致批准,患者与家属对研究方法、目的均明确了解,并签署了知情同意书。, 百拇医药(郭慧芳)
