牙合面磨耗对活髓牙牙根纵裂的影响(1)
[摘要]目的：探讨牙合面磨耗对活髓牙牙根纵裂的影响，为牙根纵裂的诊断提供帮助。方法：将900颗牙合面发生磨耗的活髓牙分级，统计各磨耗等级发生牙根纵裂的频数百分比，收集牙根纵裂患牙的临床资料，包括年龄、性别、牙位、患根分布、牙周、影像学检查等进行分析评价。采用SPSS 23.0 软件包对数据进行χ2检验。结果：①磨耗Ⅲ级和Ⅳ级发生牙根纵裂的可能性大于Ⅰ级和Ⅱ级，Ⅲ级和Ⅳ级无显著差别；②牙根纵裂多发生于40～60岁患者的下颌第一磨牙的近中颊根，男女比例为2：1。患牙大多有牙周病变，影像学检查可见牙根纵裂的患牙表现出不同程度的牙周骨质破坏、根尖周骨质破坏，有时表现为根管突然增宽等。结论：牙牙合面形态的异常改变对牙根纵裂的发生有一定影响，牙根纵裂的临床症状及影像学表现复杂多样，临床医生需加深认识，做到早诊断、早治疗，使更多的患牙得以保留。

The Influence of Abrasion on Vertical Root Fracture of the Pulp Teeth

Abstract： Objective To investigate the influence of abrasion on vertical root fracture of the pulp teeth and provide help for the diagnosis of vertical root fracture. Methods Rating 900 pulp teeth which have abrasion，and to statistics the proportions of teeth with vertical root fracture in each grade. Clinical data of the teeth with vertical root fracture were collected， which consist patients’age， sex， tooth position， with the root distribution， periodontal status and radiographic characteristics， then analysis these data. All statistical compared using χ2 test with SPSS 23.0 software package. Results Vertical root fracturewere frequently found in Level 3and Level 4， there is no significant difference in probability of Level 3 and Level 4. Vertical root fracture occurred in patients aging 40 to 60 years， the first molar mesial buccal root was mostly involved. Most teeth in the investigation have periodontal disease. Radiographic characteristics of the teeth with vertical root fracture were typically associated with periodontal bone loss， apical bone loss，some teeth show a sudden widening of the root canal. Conclusion Abnormal changes of the occlusal surface morphology have a certain effect on vertical root fracture of the pulp teeth.Thereare some predisposing factors in the occurrence of vertical root fracture， and the clinical manifestations are complex. Doctor should strengthen the understanding of the disease in order to achieve early diagnosis and early treatment， to save more teeth.

牙根纵裂(Vertical root fractures)是指牙根发生平行于牙长轴的，未波及牙冠的纵向裂纹，纵裂使得根管腔与牙周膜贯通。牙根纵裂是一种涉及牙体、牙髓、牙周组织严重的牙体疾病[1-3]。初期，患者的主观症状并不明显。随着牙根纵裂的发展，患者常常感到咬物不适，牙龈肿胀，有时出现瘘管等，患者常因此就诊。影像学检查可见患牙有牙周及根尖周骨质的破坏，根管下端突然增宽等[4-6]。临床上，活髓牙牙面发生不同程度的磨耗时，由于牙牙合面形态的异常改变常常伴随着创伤性牙合力，牙合创伤可能是活髓牙发生牙根纵裂的主要因素。本文通过研究900颗发生牙合面磨耗的活髓牙各磨耗等级发生牙根纵裂的百分比，探讨牙合面磨耗对活髓牙牙根纵裂的影响，以期对牙根纵裂的诊断提供帮助。, http://www.100md.com(赵璐 李亮 王小禾 吴佩玲)
