乙肝携带者血清HBV DNA浓度对乳汁HBV DNA的影响(1)

     【摘要】 目的 探讨乙型肝炎病毒携带产妇血清HBV DNA浓度对哺乳的影响。方法 选择乙肝携带产妇145例,采用荧光定量聚合酶链反应(FQ-PCR)技术对产妇血清HBV DNA与乳汁HBV DNA的进行检测,并将产妇血清HBVDNA浓度分级,分析产妇血清不同HBV DNA浓度级对乳汁HBV DNA阳性率。结果 当产妇血清HBV DNA<500 copies/ml时,74例产妇的乳汁中检出HBV DNA阳性为零,当产妇血清HBVDNA浓度500~1e6copies/ml时,30例产妇乳汁中检出HBV DNA阳性为3例,阳性率为10%(3/30),当产妇血清HBVDNA浓度为≥1e6copies/ml时,41例产妇乳汁中检出HBV DNA阳性为36例,阳性率为87.8%(36/41)。结论 当产妇血清 HBV DNA浓度达到或超过1e6copies/ml时,乳汁中HBVDNA阳性率大幅度增加;而当产妇血清HBVDNA浓度小于1e6copies/ml时,乳汁中HBV DNA检出率低。

    【Abstract】 Objective To investigate the effect of maternal HBV-DNA concentration on that of the breast-feeding of postpartum women with chronic HBV infection.Methods HBV DNA levels in serum and breast milk were detected by fluorescent quantitative polymerase chain reaction in 145 postpartum women with chronic HBV infection.Serum HBV DNA was graded.The effect of different grade of maternal serum HBV DNA on the breast milk was analyzed.Results 74 cases of maternal serum HBV DNA quantity were<500 copies/ml,their breast milk HBVDNA quantitywere zero.30 cases of maternal serum HBV DNA quantity ranged of 500~1.0 e6copies/ml,3cases showed positive in breast milk,the positive rate was 10%.41 cases of maternal serum HBV DNA quantity were ≥1.0 e6 copies/ml,the HBV DNA positive rate in breast milk reached to 87.8%(36 cases).Conclusion When the maternal serum HBV DNA concentration reached or exceeded 1 e6copies/ml,the positive rate of breast milk HBVDNA significant increase; and when the maternal serum concentration is less than 1 e6copies/ml HBVDNA when the HBV DNA detection rate in breast milk.
