降压调脂降糖抗血小板聚集联合治疗期间微机系统管理的临床研究(1)

【摘要】 目的 探索动脉粥样硬化性血管病降血压、调血脂、降血糖及抗血小板凝集联合治疗期间微机系统管理的作用。方法 对来自我院门急诊、住院患者及人群普查中新发现的高血压和/或血脂异常和/或血糖异常的患者，随机分为系统管理组及非系统管理组，观察生活行为、血压、血脂、血糖的及主要心血管事件的变化。结果 两组患者在治疗4周及12周时血压、血脂及血糖结果比较差异有统计学意义(P<001)；系统管理组主要心血管事件发生率较非系统管理组显著减少(P<001)。结论 降压调脂降糖抗血小板聚集药物联合治疗的微机专业管理软件可以更好地管理、分析、判断、总结以及提高治疗方案实施的准确性，可以高效、长期、全面的管理众多患者，从而使动脉粥样硬化性血管事件得到有效控制。

一直以来，我国动脉粥样硬化性血管病的发病率一直处于世界水平前列。近年来，随着我国人民生活水平的不断提高、生活方式西方化以及饮食结构不合理，高血压、高血脂及糖尿病等疾病的发病率增长趋势不减。这类疾病对机体产生的影响极大，会促使心脑血管疾病事件的发生，加速动脉粥样硬化性血管病的发生发展和死亡危险，大大增加心脑血管终点事件的发生，进而严重降低患者的生活质量和威胁患者的生命安全。为了提高这类疾病的治疗效果、降低治疗成本，本中心采用专门的软件在国内首次评估联合降血压、调血脂、降血糖及抗血小板凝集治疗期间微机管理的作用。现将研究结果报告如下。
1 资料与方法
11 一般资料
对来自我院门急诊、住院患者及人群普查中新发现的高血压病和/或血脂异常和/或血糖异常的患者采取自愿的方式，进行首诊相关检查和系统治疗管理，选择为研究对象，并定期进行检查，比较管理前后情况。高血压的诊断以2009年基层版《中国高血压防治指南》[1]为标准；血脂异常的诊断以2007年公布的《中国成人血脂异常防治指南》[2,3]为标准；糖尿病诊断标准：空腹血糖≥70 mmol/L、任意血糖≥111 mmol/L，任达一项指标即可判断为糖尿病[4]。排除标准：继发性高血压、活动性肝病不适宜调脂治疗者、活动性胃溃疡或出血性胃病不能耐受阿司匹林治疗者、对阿托伐他汀或阿司匹林过敏者及合并其他慢性消耗性疾病晚期者。所有入选患者按就诊先后顺序编号，完全随机将其分为系统管理组及非系统管理组。自2008年1月至2010年1月 ......
