超声弹性面积比法联合彩色多普勒超声阻力指数对乳腺肿瘤的诊断价值(1)

     【摘要】 目的:探究超声弹性面积比法(EAR)联合彩色多普勒超声阻力指数(RI)对乳腺肿瘤的诊断价值。方法:选取2019年1-12月于揭阳市人民医院乳腺外科行手术治疗的58例乳腺肿瘤患者为研究对象。比较良、恶性病灶EAR和RI水平,分析EAR、RI单独及联合诊断在乳腺肿瘤中的应用价值。结果:恶性病灶EAR和RI水平均明显高于良性病灶,差异均有统计学意义(P<0.05);Logistic回归分析显示,EAR和RI均是诊断恶性病灶的独立影响因素(P<0.05);EAR最佳诊断临界值为1.50,敏感度为76.92%,特异性为79.66%,AUC为0.829(95%CI:0.779,0.867);RI最佳診断临界值为0.66,敏感度为69.23%,特异性为79.66%,AUC为0.793(95%CI:0.763,0.815);EAR和RI联合诊断敏感度为96.15%,特异性为81.36%,AUC为0.908(95%CI:0.799,0.919)。结论:EAR和RI可单独应用于乳腺肿瘤诊断中,但EAR和RI联合诊断可提高诊断效能,值得临床进一步研究并推广。

    [Abstract] Objective: To investigate the diagnostic value of ultrasonic elastic area ratio (EAR) combined with color Doppler ultrasonic resistance index (RI)in breast tumor. Method: A total of 58 patients with breast tumors treated in the department of breast surgery of Jieyang People’s Hospital from January to December 2019 were selected as the observation subject. The levels of EAR and RI of benign and malignant lesions were compared, and the application value of EAR and RI alone and combined diagnosis in breast tumors were analyzed. Result: The levels of EAR and RI in malignant lesions were significantly higher than those of benign lesions, and the differences were statistically significant (P<0.05). Logistic regression analysis showed that EAR and RI were independent influencing factors in the diagnosis of malignant lesions. The best diagnostic threshold of EAR was 1.50, the sensitivity was 76.92%, the specificity was 79.66%, and the AUC was 0.829 (95%CI: 0.779, 0.867). The best diagnostic threshold of RI was 0.66, the sensitivity was 69.23%, and the specificity was 79.66%, and the AUC was 0.793 (95%CI: 0.763, 0.815). The sensitivity of EAR and RI combined diagnosis was 96.15%, and the specificity was 81.36%, and the AUC was 0.908 (95%CI: 0.799, 0.919). Conclusion: EAR and RI can be used alone in the diagnosis of breast tumors, however, EAR and RI combined diagnosis can improve the diagnostic efficiency and is worthy of further clinical study and promotion.
